Planning for Future Health: Part 2 of 2
A free virtual two-part series presented by MJHS Hospice and Palliative Care. This program is designed to help you think about and plan for your future health. You will learn to navigate the healthcare landscape and empower those you trust most to make healthcare decisions on your behalf if you can no longer speak for yourself. Planning ahead helps prevent crisis decision making and allows for peace of mind, enabling you to share quality time with those you love.

“RETAINING CHOICE IN A CHANGING HEALTHCARE LANDSCAPE”
This session will discuss steps you can take to think about ‘what you want’ and engage in conversations with those you love, leading to the completion of advance directives such as health care proxy, living will, etc. You will walk away with an understanding of what to do with the documents after they are completed.

“ACCESSING HEALTHCARE RESOURCES WHEN FACING SERIOUS ILLNESS”
This session will cover an often-misunderstood healthcare benefit and type of care that can be accessed when people face advanced or serious illness. Participants will understand how to distinguish between Hospice and Palliative Care and what the eligibility criteria are. Attendees will also learn about the comprehensive basket of services that this federal Medicare and insurance covered benefit provide in the comfort of one’s home, or wherever a person resides.
