NY Connects Brooklyn, operated by PSS and funded by NYC Aging, is a free, trusted resource that helps individuals of all ages and abilities understand and access long-term services and supports. Our goal is to empower the residents of Brooklyn to make informed decisions about their care options and maintain independent lives in their communities.

In the past year alone, NY Connects Brooklyn supported over 4,700 community members, helping them navigate complex systems of care and access vital services to remain safe, supported, and independent.

What We Do

Call or email NY Connects Brooklyn to speak with a coordinator who will assess your needs and connect you to services that fit. We offer phone consultations, virtual meetings, and in-person assistance by appointment. No eligibility criteria or income requirements.
