PSS Circle of Care’s Dementia Friendly Community (DFC) model builds the capacity of the community to better understand and support people living with dementia and their caregivers. In order to achieve this, every PSS DFC initiative targets the diverse and distinct communities that define New York City, less defined by geography but instead by ethnicity, culture or faith. Comprised of 14 steps, our model assumes a bottoms-up, grass roots approach that empowers community members to become active participants in the creation of their dementia friendly community, one where people with memory loss and their caregivers are empowered, supported and included in society.
